Welcome to Prairie Heights Middle School Athletic Registration
*Please read and follow these steps carefully to be fully registered!
Step 1: You must have a current physical exam completed by a certified professional. An exam is good for a full calendar year. Once you have your physical completed by a doctor, you can turn it into the office or you can scan it and submit completed Physical Forms in the registration tab below.
Step 2: Fill out and submit the Online Registration Form (click icon below) prior to the start of the season. Please read through these forms carefully and be certain to complete and acknowledge each one where required.
Step 3: After completion of registration you must pay a fee for middle school sports. Sports are $35/season or $70 for the year. Pay your Activity Fee online to complete your registration process. Please click HERE. Contact the Activities Office if you are unable to pay online.
